Fla. Admin. Code Ann. R. 64F-3.002 - County Health Department Responsibility
The County Health Department (CHD) is responsible for:
(1) Providing blank Healthy Start risk
screening forms to providers responsible for Healthy Start risk
screening.
(2) Receiving completed
Healthy Start risk screening forms in accordance with Rule
64C-7.009, F.A.C.
(3) Establishing an internal system for
processing screening forms in accordance with Rule
64C-7.009, F.A.C.
(4) Monitoring screening instruments for
completeness and obtaining corrections as needed before the screening data are
entered into the statewide prenatal and infant confidential registry.
(5) Sending the screening forms to the county
health department in the county of residence.
(6) Batching and sending screening forms to
the provider of care coordination within five working days.
(7) In addition, the CHD is responsible for
establishing, coordinating, and overseeing the Healthy Start care coordination
system outlined in Rule
64F-3.004, F.A.C., in the
following situations:
(a) When there is not a
local Healthy Start Prenatal and Infant Care Service Delivery Plan approved by
the department, or
(b) When there
is a local Coalition service delivery plan and the Coalition designates through
contract or memorandum of agreement the CHD as the responsible entity for all
or part of Healthy Start care coordination system
activities.
