Fla. Admin. Code Ann. R. 64J-2.015 - Process for Renewal of Trauma Centers
(1)
At least 14 months prior to the expiration of the trauma center's
certification, the department will send, by electronic mail, to the email
address of record for the hospital's chief executive officer or equivalent of
each trauma center that is eligible to renew, a blank Form DH 2032R, January
2010, Trauma Center Application to Renew, which is incorporated by reference
and available at Florida Department of Health, Office of Trauma, 4052 Bald
Cypress Way, Bin C-18, Tallahassee, Florida 32399-1738,

The completed Form DH 2032R must be returned to the department via electronic
mail or certified mail within 15 days of receipt to apply to renew the
certification.
(2) All renewing
trauma centers will receive an on-site survey after the department's receipt of
the completed Form DH 2032R. The department will notify each trauma center of
the results of the site survey within 45 days from completion of the site
survey. If the trauma center desires to provide additional information
regarding the results of the site survey to the department to be considered,
the information must be provided in writing and be received by the department
within 45 days of the trauma center's receipt of the department's notice. If
the trauma center fails to timely respond to the department's notice, the
department will make the final determination of approval or denial based solely
on information collected during the applicant's site survey.
(3) No later than 10 days prior to the
expiration of the certification, the department will notify each trauma center
in writing of its approval or denial by electronic mail to the email address of
record for the hospital's chief executive officer or equivalent. If the
department determines that the trauma center meets the requirements for renewal
of its trauma center designation, the department may issue an approval and
renewal certification immediately upon completion of the site survey process as
outlined in subsection (2) of this rule. The renewal certification will be for
a period of 7 years beginning the day after the current certification
expires.
