Fla. Admin. Code Ann. R. 6A-6.024 - School Entry Health Examination
This rule implements the school entry health examination required by Section 1003.22, F.S.
(1) Any health professional who is licensed
in Florida or in the state where the student resided at the time of the health
examination and who is authorized to perform a general health examination under
such licensure shall be acceptable to certify that health examinations have
been completed.
(2) Certification
that a health examination has been completed may be documented on the State of
Florida, Department of Health, DH Form 3040-CHP-07/2013, "School Entry Health
Exam, " which is incorporated by reference (http://www.flrules.org/Gateway/reference.asp?No=Ref-05986)
and available online at
http://www.floridahealth.gov/programs-and-services/childrens-health/school-health/_documents/school-health-entry-exam-form-dh3040-chp-07-2013.pdf,
or a signed statement by an authorized professional that indicates the results
of the components included in the health examination. A hard copy of the School
Entry Health Exam may be obtained by contacting Student Support Services,
Turlington Building, 325 West Gaines, Suite 644, Tallahassee, Florida
32399.
(3) Transfer of all student
health records shall be in accordance with paragraphs
6A-1.0955(7)(a),
(b), F.A.C.
