Haw. Code R. § 17-534-30 - Owner application

(a) An owner seeking to participate in the program shall submit an application form, as prescribed by the authority. The form shall include, but not be limited to, the owner's name, address, agent's name, ownership information, property information, unit information, and rent information.

(b) An owner shall submit documentation as required by the authority and shall execute a release of information form in order for the authority to verify the owner's eligibility.

(c) An owner shall execute an agreement that any tenant information disclosed to the owner shall be held in confidence and shall not be released to anyone in any form.

(d) In the event an owner does not have a dwelling unit that meets the requirements of section 17-534-31, then such owner may apply for a construction grant pursuant to subchapter 4.
