Haw. Code R. § 17-892.1-2 - Application
(a) A completed application to operate a
group child care center or group child care home must include the following:
(1) A signed department application
form;
(2) A written statement of
operation policies;
(3)
Verification that the facility meets the applicable county codes;
(4) Completed employment history clearance
forms;
(5) Results of the criminal
history clearance and child abuse/neglect history check as indicated in (A) and
(B) below:
(A) Applicants and employees shall
provide criminal history records, child abuse/neglect history, employment
information and consent to conduct such checks as may be required by state or
federal law. Such information and consent shall be given upon forms supplied by
the department.
(B) The department
shall conduct criminal history, employment history, and child abuse/neglect
history checks on all applicants and their employees; applicants shall conduct
employment history and background checks on prospective employees.
(b) The date of
application shall be the date a signed application form and all required
information and documentation are received by the department.
(c) Notification of the disposition of the
completed application for certificate of approval shall be issued no later than
ninety days from the date the completed application as defined in section
17-892.1-2(a) is received.
(d) If
the department fails to issue a notification of the disposition of the
application within ninety days, the application shall be deemed approved and a
license shall be issued.
