Haw. Code R. § 4-45-28 - Affidavits
Prior to the sale, offering for sale, or distribution of any feed, the person registering the feed or, in the case of a feed not subject to registration, the local distributor or importer of the feed shall submit to the department an affidavit, on a form approved by the department, attesting to the fact that the feed does not contain any pesticide residue or other adulterant in excess of the tolerances and action levels in this subchapter.
