Ill. Admin. Code tit. 92, § 1480.60 - Conversion of Class "EE" Recovery Permit to Class "E" Recovery Permit
Upon completion of an approved recovery agency employee certification program under Section 1480.160, a holder of a Class "EE" recovery permit in good standing may file an application for conversion of his or her Class "EE" recovery permit to a Class "E" recovery permit.
a) Applications for conversion
of a Class "EE" recovery permit to a Class "E" recovery permit shall be filed
on forms and contain the information prescribed by the Commission. Incomplete
or incorrect applications for conversion of a Class "EE" recovery permit to a
Class "E" recovery permit shall be refused or rejected by the
Commission.
b) Applicants for
conversion of a Class "EE" recovery permit to a Class "E" recovery permit shall
not be required to pay a conversion application fee.
c) Applications for conversion of a Class
"EE" recovery permit to a Class "E" recovery permit shall contain proof of the
Applicant's completion of an approved recovery agency employee certification
program.
d) A Class "EE" recovery
permit can only be renewed once and shall expire permanently at the end of the
renewal period.
