460 IAC 14-25-3 - Submitting a request for an impartial due process hearing

Sec. 3.

(a) An applicant, eligible individual, or individual's representative must submit a written request for an impartial due process hearing. An applicant, eligible individual, or individual's representative may submit a request for an impartial due process hearing on a form available in the local VR program office.
(b) The individual may seek assistance completing a request for an impartial due process hearing from a counselor, a CAP representative, the individual's representative, or an advocate of the individual's choice.
(c) An applicant, eligible individual, or individual's representative may withdraw his or her request for an impartial due process hearing, in writing, at any time.
