879 IAC 1-7-1 - Renewal of a manufactured home installer license

Sec. 1.
(a) A
manufactured home installer license issued under this article shall expire
January 1 of every fourth year.
(b)
To renew a license, an individual must do the following:
(1) Pay the fee required by
879 IAC 1-4-1.
(2) Complete an application for renewal on a
form provided by the board.
(3)
Satisfactorily complete the continuing education required by
879 IAC 1-8.
(4) Submit a certification or proof of
continuation of the insurance coverage or surety bond required by
879 IAC 1-5.
(5) Sign a statement under penalty of perjury
that:
(A) the hours submitted are
correct;
(B) the licensee attended
and completed the courses taken; and
(C) to the best of the licensee's knowledge,
the courses completed meet the requirements of
879 IAC 1-8.
