02-658 C.M.R. ch. 104, § 9 - Application for Examination
1. Applicants for
examination(s) must submit all of the following:
A. Completed Application;
B. Application Fee (nonrefundable);
C. School Certificate(s) (if applicable);
and
D. Affidavit(s)
2. Incomplete or illegible
applications will be returned to the applicant together with any
attachments.
3. Applicants must
receive Board approval prior to sitting for an examination. Examination scores
of applicants who take an examination prior to receiving Board approval will be
null and void.
