A.
To apply
for the DMH Licensed Administrator (LA) program, an applicant must have
a "Qualified Sponsor," as defined in Chapter Six of this document.
B. As applicable, the prescribed
LA Sponsorship Form (a component of the LA Application Form) must be completed
by a " Qualified Sponsor."
