A. Complete the
CHDO Certification Application, including all requested documentation
forms.
B. Submit one (1) original
and one (1) copy of the entire application. The application should be bound
with a binder clip, 3-ring binders or any other forms of binding.
C. The application may be mailed or hand
delivered to the address below.
