Ohio Admin. Code 4765-24-04 - Application for a fire charter

(A) An applicant for a charter to provide firefighter, fire safety inspector, hazard recognition officer, or instructor courses, issued pursuant to section 4765.55 of the Revised Code and this chapter, shall submit a completed "Fire Charter Application" to the division.

(B) There shall be no application fee for a fire charter issued under this chapter.

(C) An application that is not filled out in the manner specified may be deemed incomplete by the executive director. Incomplete applications will not be considered by the executive director.
