34 Pa. Code § 125.5 - Preliminary requirements
(a) An
applicant shall have been in business for at least 3 consecutive years prior to
application.
(b) An applicant shall
be incorporated or organized under the laws of a state of the United
States.
(c) Each employer
requesting self-insurance shall have an adequate accident and illness
prevention program.
