Utah Admin. Code R277-407-13 - Fee Waiver Reporting Requirements
(1) An LEA shall
collect the following information, which may be requested by the Superintendent
as part of the Superintendent's monitoring of the LEA's school fees practices:
(a) a summary of:
(i) the number of students in the LEA given
fee waivers;
(ii) the number of
students who worked in lieu of a waiver;
(iii) the number of students denied fee
waivers; and
(iv) the total dollar
value of student fees waived by the LEA; and
(b) the total dollar amount of all fees
charged to students within all schools within the LEA.
(2) An LEA shall submit school fee revenue
information in the Utah Public Education Financial System as provided in Rule
R277-113.
