Utah Admin. Code R382-10-9 - Social Security Numbers
(1) The
eligibility agency may request an applicant to provide the correct Social
Security Number (SSN) or proof of application for a SSN for each household
member at the time of application for the program. The eligibility agency shall
use the SSN in accordance with the requirements of
42 CFR
457.340(b), October 1, 2012
ed., which is incorporated by reference.
(2) The eligibility agency shall require that
each applicant claiming to be a U.S. citizen or national provide their SSN for
the purpose of verifying citizenship through the Social Security Administration
in accordance with Section 2105(c)(9) of the Compilation of the Social Security
Laws.
(3) The eligibility agency
may request the SSN of a lawful permanent resident alien applicant, but may not
deny eligibility for failure to provide an SSN.
(4) The Department may assign a unique CHIP
identification number to an applicant or beneficiary who meets one of the
exceptions to the requirement to provide an SSN.
