Utah Admin. Code R384-201-6 - Proof of Screening
Certificate or health form documenting a vision screening or examination given within one year of entering a public school are acceptable for school entry. A student less than age 9 entering a public school in Utah for the first time without proof of screening mentioned above, shall be screened during that school year.
