Utah Admin. Code R386-703-7 - Report Requirements: Report Destinations
(1) Except in (a), (b), and (c), an
individual or agency shall report a reportable injury to the Bureau of
Epidemiology or to the local health department responsible for the geographic
area where the injury occurred.
(a) When a
case results in death, an individual or agency reporting a reportable injury
may submit a completed death certificate to the Office of Vital Records and
Statistics in lieu of reporting to the Bureau of Epidemiology.
(b) In the case where an injury is evaluated
in an emergency department of a health care facility, the health care facility
may, in lieu of reporting to the Bureau of Epidemiology, submit a complete
emergency department log to the Bureau of Emergency Medical Services if the
records are submitted through an electronic medium and format acceptable to the
Bureau of Emergency Medical Services.
(c) In the case where a reportable injury
listed in Section
R386-703-3 is
reported under the requirements of the Utah Health Data Authority Act, 26-33a,
the individual or agency may:
(i) grant
permission to the Department in writing that information relating to a case
with a reportable injury may be supplied to the Bureau of Epidemiology before
the identifying information is removed from the report; or
(ii) report the injury both in accordance
with the Utah Health Data Authority Act, 26-33a, and to the Bureau of
Epidemiology.
(2) The local health officer shall forward
each original report to the Bureau of Epidemiology. The local health department
may maintain copies of these reports.
