Utah Admin. Code R398-15-5 - Criteria for Awarding a Grant
(1) Providers
shall submit a funding request to the advisory committee on a standard request
for application form which will be developed and authorized by the advisory
committee and made available by the department.
(2) An individual may apply for a grant from
the program by submitting an application to a qualified provider.
(3) The department shall transmit a grant
awarded on behalf of an applicant to a qualified provider designated by the
applicant.
(4) The advisory
committee shall consider the availability of funds in the Adult Autism
Treatment Account before awarding a grant.
(5) The department shall create a file for
the qualified individual that shall include:
(a) the application;
(b) related medical and service
documentation;
(c) correspondence
from the qualified provider, qualified individual, and the
department;
(d) report on the
evaluation of benefits and outcomes for individuals receiving services from the
qualified provider;
(e) the
approved grant and supporting documentation; and
(f) the file and documentation will be stored
securely for 10 years after the completion of the grant and then be
destroyed.
(6) The
advisory committee may review and determine if the qualified provider is
ineligible to receive funds when:
(a) the
qualified provider's approved services are found to be inactive for two
years;
(b) the qualified provider
is no longer providing evidence-based treatments;
(c) the department has made three documented
attempts to contact the responsible party or recipient to discuss inactivity
without response;
(d) there is a
violation of the terms of the executed grant; or
(e) the qualified individual is no longer a
Utah resident.
