Utah Admin. Code R428-12-5 - Sampling Frame Submission
(1) The
sources for enrollment data are carriers. Each carrier shall include any
eligible enrollee in the sampling frame.
(2) Each carrier shall create the sampling
frame according to the criteria outlined in the National Committee for Quality
Assurance Survey Specifications, incorporated by reference in Subsection
R428-1-4(2).
(3) The department may audit the sampling
frame and any procedure used by the reporting carrier.
(4) Each carrier shall copy the sampling
frame using an electronic medium acceptable to the survey agency and submit the
copy to the survey agency.
(5) Each
carrier shall fill out the Sample Description sheet provided by the survey
agency and submit the sheet with the electronic sample file. Each carrier shall
submit to the survey agency the sampling frame for each of its products no
later than four weeks after the receipt of the sampling memo from the survey
agency.
