Utah Admin. Code R432-151-22 - Quality Assurance
(1) This section
shall apply in addition to
R432-150-11.
(2) The quality, appropriateness, and scope
of services rendered shall be reviewed and evaluated on at least a quarterly
basis by an interdisciplinary quality assurance committee.
(3) A written report of findings from each
meeting shall be submitted to the administrator and shall be available for
review by the Department.
(4)
Committee composition.
(a) Members of the
quality assurance committee shall be appointed by name in writing by the
administrator for a given term of membership.
(b) The committee shall have a minimum of
three members with representation from at least three different licensed health
care professions.
(5)
Methodology for evaluation includes:
(a)
Review and evaluation of active and closed resident records to assure that
established policies and procedures are being followed;
(b) Facility policy and procedure will
determine the method(s) to be followed for selection and review of the
representative sample of active and closed records;
(c) Review and evaluation whether needed
services were provided;
(d) Review
and evaluation of coordination of services whenever appropriate through
documentation of written reports, telephone consultation, or case conferences;
and
(e) Review and evaluation of
the resident plans of treatment for content, frequency of updates, and whether
progress notes correspond to goals stated in the resident care plan.
