Utah Admin. Code R432-153-5 - Scope of Services
(1) The facility
shall provide 24-hour licensed nursing services.
(a) The facility shall ensure that nurses are
present on the premises at all times to meet the needs of clients.
(b) The facility shall provide at least one
registered nurse either by direct employ or by contract to act as the director
of nursing.
(c) The facility shall
employ a registered nurse for at least eight consecutive hours a day, seven
days a week.
(d) A licensed nurse
shall serve as charge nurse on each shift.
(2) The facility shall provide medical
supervision and dietary services.
(3) The following services shall be provided
as required in the client service agreement:
(a) physical therapy;
(b) occupational therapy;
(c) speech therapy;
(d) respiratory therapy; and
(e) other therapies as indicated.
(4) The facility shall coordinate
the delivery of respite services with the recipient of services, the case
manager if one exists, and the family member or primary caregiver.
(5) The facility shall document the client's
response to the respite placement and coordinate with each provider agency to
ensure an uninterrupted service delivery program.
(6) The facility shall complete a service
agreement to serve as the plan of care, which shall identify the prescribed
medications, primary care provider treatment orders, need for assistance with
activities of daily living and diet orders.
(7) The facility shall have access to
supplemental services such as laboratory and pharmacy as needed.
