Utah Admin. Code R432-270-13 - Resident Assessment
(1) The licensee
shall ensure a signed and dated resident assessment is completed for each
resident before admission and at least every six months thereafter.
(2) In type I and type II facilities, a
licensed health care professional shall complete and sign the initial and
six-month resident assessment.
(3)
The licensee shall ensure that the resident assessment:
(a) accurately reflects the resident's status
at the time of assessment; and
(b)
includes a statement signed by the licensed health care professional completing
the resident assessment that the resident meets the admission and level of
assistance criteria for the facility.
(4) The licensee shall ensure the resident
assessment form is approved and reviewed by the department to document the
resident assessments.
(5) The
licensee shall ensure each resident's assessment is revised and updated when
there is a significant change in the resident's cognitive, medical, physical,
or social condition and update the resident's service plan to reflect the
change in condition.
