Utah Admin. Code R432-300-6 - Criteria for Type N Facility
The licensee shall ensure the following criteria are met to obtain a license for a Small Health Care Facility - Type N:
(1) provide care in a residence where the
licensee lives full time;
(2) meet
local zoning requirements to allow the facility to be operated at the given
address;
(3) obtain a certificate
of fire clearance annually from the local fire marshal having
jurisdiction;
(4) have a physician
assessment and approval for each resident's admission;
(5) provide daily, licensed nursing care;
and
(6) provide 24-hour direct care
staff available on the premises.
