Utah Admin. Code R432-40-5 - Reporting of Data
(1) By April 30 of
each year, each licensee shall report to the DHHS Immunization Program:
(a) the number of residents in their facility
during the reporting period who received an influenza vaccination at any time
during the current influenza season, even if the residents are no longer in the
facility;
(b) the number of
residents in their facility during the reporting period who have ever received
a pneumococcal vaccination, even if the residents are no longer in the
facility;
(c) the number of
residents in their facility during the reporting period who received a COVID-19
vaccination according to ACIP recommendations, even if the residents are no
longer in the facility;
(d) the
number of employees on staff during the reporting period who received an
influenza vaccination at any time during the current influenza season, even if
the employees are no longer employed by the facility at the time of reporting;
and
(e) the number of employees on
staff during the reporting period who received a COVID-19 vaccination according
to ACIP recommendations, even if the employees are no longer employed by the
facility at the time of reporting.
(2) By April 30 of each year, each licensee
shall report to the DHHS Immunization Program to the DHHS Immunization Program:
(a) the number of residents in their facility
during the reporting period who declined an offered influenza vaccination, even
if the residents are no longer in the facility;
(b) the number of residents in their facility
during the reporting period who declined an offered pneumococcal vaccination,
even if the residents are no longer in the facility;
(c) the number of residents in their facility
during the reporting period who declined an offered COVID-19 vaccination, even
if the residents are no longer in the facility;
(d) the number of employees on staff during
the reporting period who declined an offered influenza vaccination, even if the
employees are no longer employed by the facility at the time of reporting;
and
(e) the number of employees on
staff during the reporting period who declined an offered COVID-19 vaccination,
even if the employees are no longer employed by the facility at the time of
reporting.
