Utah Admin. Code R432-500-4 - Licensure
(1) A license is required to operate a
freestanding ambulatory surgical center in accordance with Rule
R432-2.
(2) Exempt facilities are
outlined in Section
26B-2-205. Physician-based
surgical centers shall request an exemption to this rule to apply for Medicaid
or Medicare certification.
