Utah Admin. Code R432-600-12 - Contracts
(1) The licensee shall make arrangements for
professional and other required services not provided directly by the facility.
If the facility contracts for services, there shall be a signed, dated
agreement that details all services provided.
(2) The contract shall include:
(a) The effective and expiration
dates;
(b) A description of goods
or services to be provided;
(c)
Copy of the professional license, if applicable.
