Utah Admin. Code R432-650-8 - Patient Care Plan
(1) Each patient
must have a care plan that is developed and implemented by the
interdisciplinary team with the patient's consent within one month of beginning
treatment.
(2) Each patient who
receives treatment for more than 90 days must have a long-term care program
that is developed and implemented by the interdisciplinary team with the
patient's participation.
