Utah Admin. Code R434-50-4 - Criteria for Awarding Grants
The Department shall consider:
(1) the extent to which the applicant:
(a) demonstrates that it will provide
assistance to the greatest number of persons with bleeding disorders residing
across the state ;
(b) utilizes
other sources of funding, including private funding, to provide bleeding
disorder services; and
(c)
provides:
(i) information that meets the
requirements established in Subsection
26-47-103 (3);
(ii) a description of the individuals to be
served by the grant;
(iii) the
estimated number of individuals to be served with the grant award;
and
(iv) the results of an
assessment of need demonstrating the need for the bleeding disorder services
that the grantee proposes to provide;
(2) the cost to the person with a bleeding
disorder for the bleeding disorder services;
(3) the degree to which the applicant meets
the requirements of the statute; and
(4) the degree to which the application is
feasible, clearly described, and ready to be implemented.
