Utah Admin. Code R510-104-11 - Contribution Policy
(1) The actual cost
of a congregate meal and a suggested contribution shall be posted at the
congregate meal site.
(2) Each
eligible individual shall have an opportunity to voluntarily and anonymously
contribute toward the cost of a provided meal service.
(a) Each local area agency shall establish
and implement procedures to protect the privacy of an individual's decision to
contribute toward the cost of a provided meal service.
(b) A locked contribution box shall be placed
at the congregate meal site in an area that protects the privacy of the
individual's decision to contribute toward the cost of the provided meal
service.
(c) Any contributions
shall be counted by two persons, and both individuals shall sign a form
attesting to the amount received. Such documentation shall be maintained by the
local area agency.
(3) An
individual that does not meet the eligibility criteria described in Section
R510-104-4 shall pay the full
cost of the meal, which shall be collected and accounted for separate from any
voluntary contribution.
