Utah Admin. Code R510-400-15 - Purchase and Rental of Equipment
(1)
A local area agency may purchase or rent equipment if it is deemed necessary
for the individual's care and no other funding source is available.
(2) Purchased and rented equipment shall be
the property of the local area agency.
(3) Each local area agency shall develop
policy and procedures that address the disposition, inventory, and repair of
equipment.
(4) An individual or the
individual's agent sign shall sign an agreement that requires the equipment to
be returned to the local area agency when the equipment is no longer
needed.
(5) Use of equipment shall
be reviewed by the case manager as part of the quarterly review to assess the
need for continued use and condition of equipment.
