Utah Admin. Code R510-400-9 - Care Planning
(1) Each case
manager shall develop an individual's care plan based upon the individual's
current situation and needs as identified in the individual's
assessment.
(2) A care plan shall
be drafted on a form approved by the division.
(3) A care plan shall be developed with the
individual's input.
(4) A care plan
shall include a description of the services to be provided, methods for the
implementation of services, the amount and frequency of the services being
provided, and the funding source of the services to be provided.
(5) A care plan shall be signed and dated by
the individual or the individual's agent, the case manager, and a registered
nurse if a registered nurse completed an assessment of the
individual.
(6) A care plan shall
be updated at least annually at the time of reassessment.
(7) A care plan shall include any formal or
informal support persons.
(8) A
care plan shall be given to the individual and shall be maintained in the
individual's client file.
(9) A
care plan shall authorize services at the minimum level and for the least
amount that will adequately meet the individual's needs.
(10) A copy of the care plan or an agency
service authorization form shall be provided to the service provider.
