Utah Admin. Code R65-13-6 - Membership Revocation and Termination
(1) Program membership may be revoked if the
member company:
(a) no longer meets the
qualifications for membership;
(b)
violates any applicable statute or rule;
(c) violates any agreement made between the
department and the member company;
(d) acts in a manner that may damage the
reputation of the program.
