Utah Admin. Code R66-30-15 - Renewal
(1) A licensee shall resubmit the documents
required in Section R68-25-4, with updated information, before December 31st of
the current year.
(2) The
department may deny a renewal for an incomplete application.
(3) The department may deny renewal for any
licensee who has violated any portion of this rule or state law.
