Utah Admin. Code R66-5-18 - Change in Operating Plans
(1) A
medical cannabis pharmacy shall submit a notice, on a form provided by the
department, before making any changes to the pharmacy's operating plan,
including:
(a) ownership or financial backing
of the facility;
(b) the facility's
name;
(c) any modification,
remodeling, expansion, reduction, or physical, non-cosmetic alteration of a
facility;
(d) change to the
protected areas; and
(e) any other
information requested by the department.
(2) Pursuant to Subsection
4-41a-201.1(7), a medical cannabis
pharmacy may not implement changes to the initial approved operation plan
without board approval.
(3) The
department shall specify the reason for the denial of approval for a change to
the operation plan.
