Utah Admin. Code R708-48-7 - Procedure to Apply for or Renew a Service Center License
(1) An applicant shall complete an
application for a service center license and mail it to the division at Box
144501 Salt Lake City, Utah 84114.
(2) The packet under Subsection (1) shall
include:
(a) an application form provided by
the division, which has been signed by the applicant and any other required
parties;
(b) an application or
renewal fee, which shall be made payable to the department; and
(c) a copy of the license to operate in the
city or municipality where the service center is located, or a statement that a
license is not required in that location.
(3) A division representative:
(a) shall inspect the location before
approval of the license; and
(b)
may make any recommendations for changes to become fully compliant for
licensure.
(4) A provider
may not transfer a service center license.
(5) A provider may obtain a duplicate service
center license by submitting a written request and a duplicate license fee to
the division.
(6) The division may
not process an application if any requirement is not met.
