Utah Admin. Code R714-562-7 - Applications
Applications for the funding of early intervention systems shall:
(1) be made on department
forms, or other forms if they contain the information included on department
forms;
(2) include criteria required
under Section
53-14-203; and
(3) meet minimum standards as required by the
department under Section
R714-562-11.
