Ariz. Admin. Code § R9-19-313 - Duties of Persons in Charge of Place of Final Disposition
A person in charge of a place of final disposition in this state shall:
1. Maintain a copy of the
following documents at the place of final disposition for at least five years
after the issue date on the document:
a. The
disposition-transit permit for each final disposition of human remains,
and
b. The disinterment-reinterment
permit for each disinterment or reinterment of human remains; and
2. Provide a copy of the document
to the State Registrar for review within 48 hours after the State Registrar's
request.
