Fla. Admin. Code Ann. R. 64B8-51.007 - Fees for Licensure Applications, Initial Licensure, and Inspection

(1) License application fee is $100.

(2) License application fee for facility license is $100.

(3) Initial licensure fee is $100 and a $5 special fee to fund efforts to combat unlicensed practice.

(4) Inspection fee for facility is $100 biennially.
