105 CMR, § 803.610 - Grounds for Denial of Certification
(1)
The Department may deny certification on any of the following grounds:
(a) Failure to conform to the application
requirements of
105
CMR 803.210,
105
CMR 803.220, and
105
CMR 803.230;
(b) Any actions or omissions which would
indicate the health or safety of the public would be at risk should
certification be granted;
(c) Any
previous violation of 105 CMR 803.000; or
(d) Any attempt to serve as a Certified
Health Officer or to obtain certification through fraud, deceit, or knowing
submission of inaccurate data or omission of a material fact.
(2) Conditions for reapplication
shall be specified by the Department at the time of the denial of
certification.
