Utah Admin. Code R380-65-8 - Record Keeping for Standing Prescription Drug Orders
(1) A person authorized to dispense
medication in the standing order shall make and retain a record that includes
the following information:
(a) the name, age,
address and contact information of each patient receiving the
medication;
(b) the NDC, or other
identifying code, of the medication dispensed; and
(c) any other relevant information required
by the standing order issued by the medical director or a physician designated
by the executive director.
(2) A person authorized to distribute
medication shall make and retain a record that includes the name, address and
contact information of each entity receiving the medication and any other
relevant information required by the department at the time of
distribution.
(3) If the
circumstances of the emergency make it impossible to comply with this section,
the executive director of the department may grant an exception in writing to
this requirement and limit the record keeping requirement to records as are
appropriate and possible in the circumstances of the emergency.
(4) Record keeping shall be in effect as
required by Section
R156-37-602 if the
executive director of the department does not grant an exception as described
in Subsection R380-65-8(3).
