Utah Admin. Code R436-2-4 - Relinquished Newborn Child Certificates
(1)
In accordance with Subsection
62A-4a-802(2)(d),
the hospital that receives a relinquished newborn child shall prepare and file
a certificate of live birth or a foundling certificate of live birth with the
Office.
(2) Determination that the
infant was safely relinquished in compliance with the Safe Relinquishment of a
Newborn Child requirements at Section 62A-4a-802 rests with the Division of
Child and Family Services.
