Utah Admin. Code R436-3-2 - Definitions
(1)
(a)
"Amendment to a vital record" means a change made to a field that appears on
the printed birth certificate or a change made a year or more after the
event.
(b) Documentary evidence
supporting the amendment may be required.
(2)
(a)
"Correction to a vital record" means a change made to a field that does not
appear on the printed birth certificate, such as birth weight or residence, or
a change to information that was entered incorrectly.
(b) Corrections must occur within one year of
the event or before the issuance of a certificate.
(c) Documentary evidence supporting the
correction may be required.
(3)
(a)
"Court order" means an order from a court of competent jurisdiction; and
(b) includes a civil petition
invoking the jurisdiction of a court of record.
(4) "Delayed Certificate of Birth" means the
certificate from a Delayed Birth Registration as defined in Section
26B-8-108.
(5) "Department" means the Department of
Health and Human Services.
