Utah Admin. Code R436-5-4 - Court Determination of Paternity
(1)
(a) A child whose parentage has been
determined by court order, or their legal representative, may have their birth
registration information updated and obtain a supplementary birth certificate
by presenting a certified copy of the court order to the state registrar along
with the required fee. The state registrar shall update the birth registration
information and prepare the supplementary birth certificate with the full name
of the child as specified in the court order.
(b) If the court order does not specify the
name to be placed on the supplementary birth certificate, the state registrar
shall leave the name as listed in the current birth registration.
(2) If the court order does not
specify the name to be placed on the supplementary birth certificate and if the
current birth certificate does not list the name of the child, the state
registrar shall:
(a) give the child or child's
legal guardian the option of returning to the court to have the name included
in the order; or
(b) if the child
is under one year of age, give the child's legal guardian the option to use the
affidavit process to add or change the child's name.

The Registrar shall prepare the new birth certificate with the full name of the person as specified in the court order. If the court order does not specify the name to be placed on the birth certificate, the State Registrar shall prepare the new birth certificate with the name as listed on the original birth certificate.
(2) If the court order does not specify the name to be placed on the birth certificate and if the original birth certificate does not list the name of the person, the State Registrar shall prepare the new birth certificate without a name. A person whose parentage has been determined by court order and whose new birth certificate does not list the child's name, or their legal representative, may seek amendment of the new birth certificate Rule R436-3.
