Utah Admin. Code R436-5-5 - Adoption

(1) Unless the court order of adoption or report of adoption specifies that a supplementary birth certificate is not to be prepared, the state registrar shall prepare a supplementary birth certificate for a child born in this state upon receipt of:
(a) a court order of adoption or a court-certified report of adoption; and
(b) payment of the required fee.
(2)
(a) Acceptable court orders are original court orders with the seal of the court or certified digital court orders.
(b) Photocopies of court orders are not acceptable.
(3) The court order shall specify the full name of the child to be entered on the supplementary birth certificate.
(4) A court order of adoption for a foreign born child may also serve as a court order for a delayed registration of birth.
