Utah Admin. Code R510-400-10 - Record Keeping
(1) Each individual
receiving HCBA services shall have a client file that includes the individual's
eligibility determination, assessment of the individual's needs, care plan,
quarterly reviews, progress notes, and any relevant legal documents.
(2) Records generated by the Alternatives
Program shall be retained and released in accordance Title 63G, Chapter 2,
Government Records Management Act (GRAMA).
(a) Except as provided in Subsection
R510-400-10(2)(c),
records generated by the Alternatives Program shall be classified as private as
defined in Section
63G-2-302.
(b) Records that contain medical,
psychiatric, or psychological content shall be classified as controlled as
defined in Section
63G-2-304.
(3) Each individual's client file shall be
secured in a locked file by the local area agency or service
provider.
(4) Records generated by
the Alternatives Program shall be kept for five years following termination of
services or until any audits initiated within five years following termination
of services have been completed, whichever is later.
