Fla. Admin. Code Ann. R. 64B-3.009 - Request for Waiver from Electronic Prescribing Requirement

(1) A practitioner authorized in this state to prescribe medicinal drugs that maintains a system of electronic health records or who prescribes medicinal drugs as an owner, employee or contractor of a licensed health care facility or practice that maintains such a system may request a waiver of the electronic prescribing requirement on form DH5042-MQA-10/2019, Request for Waiver Electronic Prescribing Requirement, incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-12148, or from the department at Department of Health, 4052 Bald Cypress Way, Bin C-11, Tallahassee, Florida 32399-1708.

(2) Waivers, if granted, cannot exceed one year in duration.
